呼吸机所致肺损伤治疗的研究进展 被引量:4

摘要 呼吸机相关性肺损伤机制主要有机械伤和生物伤两种假说,针对这两种学说发展出各种治疗措施,包括保护性肺通气策略和调节气道炎症的治疗措施,前者主要着眼于改善患者呼吸系统力学指标,后者主要作用于呼吸系统各种炎性细胞,作用于细胞内信号转导通路,调节炎症反应。这些措施的部分疗效已经在临床应用中得到证实,探讨呼吸机相关性肺损伤的机制和治疗措施,防止伤害性机械力作用于肺部并进一步阻断细胞内的传导和细胞内信号转导途径的激活,减少呼吸系统炎症细胞的激活和炎症介质的表达,对于预防呼吸机相关性肺损伤和降低危重症患者的死亡率有重要的临床意义。
