血浆CRP/APN比值与儿童急性胰腺炎Ranson评分的相关性

RELATIONSHIP OF PLASMA RATIO OF CRP/APN WITH RANSON RATE IN CHILDREN WITH ACUTE PANCREATITIS

目的评估血浆CRP/APN比值与儿童急性胰腺炎Ranson评分相关性。方法收集78例确诊急性胰腺炎患儿,依据Ranson评分分为轻型胰腺炎(42例)及重型胰腺炎(36例)组。于入院时采集并比较两组患儿血浆C-反应蛋白及脂联素水平,采用Pearson或Spearman相关分析血浆C-反应蛋白、脂联素水平及CRP/APN比值与Ranson评分相关性。结果与重型胰腺炎组相比,轻型组血浆C-反应蛋白水平和CRP/APN比值均较低(p<0.05),而脂联素水平则较高(p<0.05);血浆C-反应蛋白及CRP/APN比值与Ranson评分呈正相关,相关系数分别为0.846(p<0.05)和0.911(p<0.05),而脂联素水平与Ranson评分呈负相关(r=-0.823p,<0.05)。结论血浆CRP/APN与Ranson评分呈正相关,能够较好的反应儿童急性胰腺炎的严重程度。

Objective To investigate the relationship of plasma ratio of CRP/APN with Ranson rate in children with acute pancreatitis（AP）. Methods 78 children defined as acute pancreatitis were enrolled and assigned into mild AP（n=42） and severe AP group（n=36） according to Ranson score system.Plasma levels of C-reactive protein（CRP） and adiponectin（APN） were evaluated and compared between the two groups at initial administration.Relationship of plasma levels of CRP,APN and CPR/APN ratio with Ranson score system were evaluated by Pearson or Spearman correlation analysis. Results Compared to severe group,the plasma levels of CRP and CRP/APN ratio were significantly lower（p0.05）,while plasma level of APN was significantly higher（p0.05） in mild group.Plasma levels of CRP and CRP/APN ratio were positive correlated with Ranson score system,and correlation coefficiency was 0.846（p0.05） and 0.911（p0.05） respectively,while APN was negative correlated with Ranson score system（r=-0.823,p0.05）. Conclusion Plasma ratio of CRP/ANP was positive correlated with Ranson score system and had a good predictive value on the severity of AP in children.
