自体动静脉内瘘血流量与发生内瘘血栓的相关性分析 被引量:3

摘要 目的通过监测长期血液透析患者的自体动静脉内瘘(autogenous arterio-venous fistulas,AVF)的血流量,探讨其与AVF血栓形成的相关性。方法选取在华北石油总医院透析中心进行维持性血液透析患者84例,使用Transonic HD02血液透析监护仪(超声稀释技术)检测AVF血流量,随访6个月,以发生内瘘血栓为终点事件,将患者分为血栓组和非血栓组,分析AVF血流量与发生内瘘血栓的相关性;根据AVF血流量分将患者分为2组:高流量组(54例,Qa≥500ml/min)和低流量组(30例,Qa<500ml/min),分析2组发生血栓的危险性因素。结果结果20.2%患者随访过程中发生AVF血栓,AVF血流量在血栓组明显下降(P=0.001);低流量组半年内AVF的累计通畅率是77.4%,其发生血栓的风险是高流量组的4.5倍。结论 AVF透析患者AVF血流量越低,AVF发生血栓的可能性越大。AVF的血流量低于500ml/min是发生近期血栓的强预测因素。
