Professor Zhang Shiqing's experience in the treatment of children's eczema

通过2则医案,介绍张士卿教授运用不同方药治疗小儿湿疹的经验。张教授认为小儿湿疹多由脾失健运、湿热内蕴、血虚风燥,又兼受风邪所致。内外两邪相搏,风湿热邪浸淫血脉,使之内不得疏泄,外不得透达,郁于肌肤腠理之间,故皮疹色红瘙痒,干燥起皮或津水流溢。临证时分为干性和湿性湿疹2种类型,分别以清热利湿、祛风止痒和养血润燥、祛风止痒为主要治则。干性湿疹选用四物汤加减治疗,取其养血润燥、祛风止痒之意;湿性湿疹选用四妙丸加减治疗,取其清利湿热、祛风止痒、燥湿健脾之意。

Through two medical cases,this paper introduced Professor Zhang Shiqing＇s experience in the treatment of children＇s eczema by using different herbs.Professor Zhang thought that children＇s eczema was usually caused by spleen deficiency,inside damp heat,blood deficiency and dry wind,as well as by pathogenic wind.Pathogenic elements combating both inside and outside,wind,dampness and heat invaded into blood,so there was lack of catharsis both inside and outside,and the disease assembled on the skin.Thus the patient developed eczema,with red color,itchy rash,dry skin or spilling liquid.The symptoms could be divided into two types of dry and wet eczema.The treatment principles lied in clearing heat and dampness,expelling wind itching,flourishing blood and moistening dryness.Dry eczema could be treated by Si Wu Decoction to nourish blood,moisten dryness,and expel wind itching.Wet eczema could be treated by Si Miao Decoction with addition or subtraction in the ingredients to clear dampness and heat,expel wind itching,moisten dryness and strengthen spleen.
