摘要
目的采用基于体素的形态学分析(VBM)方法,分析原发帕金森病患者全脑灰质结构与健康人的差异。方法应用T1WI三维脑容积扫描序列对35名帕金森病患者和20名健康者对照进行扫描,获得全脑结构图像。采用以SPM5为基础的VBM方法进行数据处理得到全脑灰质图结果相比正常对照组,帕金森病患者出现广泛脑区灰质密度的减低,范围累及双侧颞叶、额叶、顶叶、枕叶,边缘系统的右侧海马、海马旁回,右侧小脑后叶及左侧尾状核。结论帕金森病患者脑灰质结构发生改变的区域广泛。VBM技术可在活体状态客观评价脑内结构的早期变化,是研究脑内形态学改变的首选方法。
Objectives To analyze the structural changes of whole brain gray matter in Parkinson's disease from voxel-based morphometry (VBM). Methods 35 Parkinson's disease patients and 20 normal controls were examined by using TIWI three-dimensional brain volume sequence to obtain the whole brain structural images. The images were analyzed by using VBM based on SPM5 to generate gray matter density map. Results Compared with normal controls, Parkinson's disease patients showed extensive decrease in gray matter density in widespread brain regions involving bilateral temporal, frontal, parietal, occipital lobes, right hippocampus, right parahippocampus gyrus, right cerebellar hemisphere and left caudate nucleus. Conclusion Gray matter atrophy in widespread brain region can exist in Parkinson's disease patients, which verifies that Parkinson's disease pathological damage can affect many different brain areas and can manifest variety symptoms. VBM can be used to evaluate the early structural changes of full-brain in vivo and is the first choice for the study of brain morphology.
