动力髋螺钉治疗老年股骨粗隆间骨折并发退钉分析

Analysis of complications of femoral intertrochanteric fracture of aged people with dynamic hip screw

目的分析动力髋螺钉(DHS)内固定治疗老年股骨粗隆间骨折并发退钉的原因,探讨相关解决方法。方法对120例老年股骨粗隆间骨折采用DHS内固定术后并发螺钉退出11例病例进行回顾分析。结果失败因素与骨质疏松程度、骨折类型、内固定选择、植入内固定的位置有关。结论骨质疏松和骨折严重程度,内固定物自身的不足是导致动力髋螺钉失败的主要因素。严格掌握DHS的手术适应证,严格规范手术操作,正确的康复锻炼可减少失败发生。

Objective To analyze the reasons of complications of femoral intertrochanteric fracture of aged people with dynamic hip screw,and discuss the methods to resolve screw relax of complications.Methods Retrospective analysis was carried out in 120 elderly patiens with femoral intertrochanteric fracture,who had been operated by dynamic hip screw.Eleven cases who had postoperative screw relax of complications were carried out by correlation analysis.Results The main factors resulting complications are correlative with the bone fracture types,grade of osteoporosis,the choice of internal fixation plant and site of place internal fixation plant.Conclusion The main factors resulting complications are the bone fracture types,osteoporosis,defect of internal fixation plant.The methods of preventing complications are： mastery of operation indication,normative operation,exact rehabilitative exercise.
