危重症机械通气患者被动等待法结合盲插法放置螺旋型鼻肠管安全性

Study of the security of critical patients with invasive ventilation adopting passive waiting and blind intubation methods placing spiral type nasojejunal feeding tube

目的探讨危重症机械通气患者床边被动等待法结合盲插法放置螺旋型鼻肠管进行十二指肠后喂养的安全性。方法对30例危重症机械通气患者采用盲插法和被动等待法放置螺旋型鼻肠管,同时给予甲氧氯普胺促进胃肠蠕动,0、24 h以后分别行pH值和床边X线腹部摄片明确管端位置,管端位于幽门后为置管成功,确定置管成功后进行喂养。结果置管总成功率76.3%,置管期间无消化道穿孔、出血等并发症,喂养期间不良事件发生率8.7%。结论通过盲插法和被动等待法相结合,危重症机械通气患者放置螺旋型鼻肠管成功率较高,进行十二指肠后喂养安全性好。

Objective To study the reliability of critical patients with mechanical ventilation adopting passive waiting and blind intubation method placing spiral type nasojejunal feeding tube bedside for nasoduodenal feeding.Methods 30 patients with invasive ventilation were placed spiral nasojejunal feeding tube adopting passive waiting and blind intubation methods,metoclopramide were given to stimulate gastrointestinal peristalsis.Feeding was started when the tubes were placed successfully.Results 76.3% of the patients were successfully placed and there was no digestive tract perforation or gastrointestinal hemorrhage occurred in the period of post nasoduodenal feeding.Adverse events were observed in 8.7% of the patients during the feeding.Conclusion Placing spiral type nasojejunal feeding tube bedside adopting passive waiting and blind intubation methods is highly successful and nasoduodenal feeding shows good security.
