经椎弓根椎体撑开器的研制与实验手术

摘要 目的探讨采用经椎弓根椎体撑开器治疗胸腰椎骨折的价值。方法选取4具人胸腰椎标本(T_9~L_5)截成10个脊柱功能节段,按Dennis分类,将标本造成前屈位压缩性骨折,再将标本随机分为两组,实验组采用自行设计的经椎弓根椎体撑开器对骨折椎体复位,复位后再注入骨水泥,对照组单纯行椎体成形术。测定术前、术后骨折椎体前缘高度、Cobb角和骨水泥注入量。结果实验组和对照组均能顺利完成整个手术过程,实验组注入的骨水泥量为(3.9±0.14)mL,对照组(2.8±0.26)mL(P<0.01)。实验组术前、术后椎体高度、后Cobb角观察结果差异有显著性意义(P<0.01),PVP组差异无显著性意义(P>0.05)。结论自主研制的经椎弓根椎体撑开器在椎体压缩骨折中能有效地撑开压缩椎体,值得推广。 Objective To investigate the value of the self-designed spine brace appliance in the vertebral body. Methods Four cadaveric spines were dissected from T9 to L5,and divide them to 10 function parts,performed the middle vertebrae fracture,and randomly separate them into two groups(A group and B group).A group adopts the self-designed spine brace appliance to replace the fracture,and then inject the bone cement.B group was only treated by vertebroplasty.Measure the anterior vertebrae height,the Cobb’s degree and the capacity of bone cement.Results Two groups complete the operation.A group injects(3.9±0.14) mL bone cement,more than B group(2.8±0.26) mL(P〈0.01).A group had a significant difference compared with preoperative in the anterior vertebrae height and the Cobb’s degree(P〈0.01).B group had no significant difference(P〉0.05).Conclusion The self-designed spine brace appliance can effectively sustain the injured vertebrae,deserves the clinical expansion.
