小切口白内障摘除术中小范围后弹力层脱离的原因及处理

Cause and treatment of Coreal descemet's membrane detachment in small incision cataract surgery

目的探讨小切口白内障囊外摘除联合人工晶状体植入术中小范围后弹力层脱离发生的原因及处理措施。方法回顾性分析204例(214眼)小切口白内障囊外摘除联合后房型人工晶状体植入术的患者术中发生后弹力层小范围脱离患者的临床资料、处理及预后。结果本组术中小范围后弹力层脱离的发生率为5.14%(11/214)。发生后弹力层脱离的患者平均77.2岁;平均术前眼压为8.3mmHg。结论高龄、较低的术前眼压是小切口白内障术中发生后弹力层脱离的高危因素,及时发现并正确处理小范围后弹力层脱离是防止大面积脱离的关键。

Objective To explore the cause and treatment of small area corneal descemet membrane detachment in surgery of small incision extracapsular cataract extraction and IOL.Methods The clinical data,treatment and prognosis of 204 patients （214 eyes） with small area corneal descemet membrane detachment in surgery of small incision extracapsular cataract extraction and IOL were retrospectively analyzed.Results The rate of small area corneal descemet membrane detachment is 5.14% （11/214）.The average age of patients with small area corneal descemet membrane detachment is 77.2 years old.The average intraocular tension before surgery is 8.3 mmHg.Conclusion Old age and lower intra-ocular tension before surgery are the high risk factors of small area corneal descemet membrane detachment in surgery of small incision extracapsular cataract extraction.Early diagnosis and treatment of small area corneal descemet membrane detachment is the key to prevent extensive area detachment.
