网织红细胞参数及血清转铁蛋白受体在青春期女性铁缺乏患者的变化 被引量:5

摘要 目的研究青春期女性铁缺乏患者的网织红细胞参数和血清转铁蛋白受体(sTfR)的变化。方法 12~15岁女性279人分成4组;检测各组的网织红细胞参数和sTfR,比较各组的指标变化以及sTfR与网织红细胞参数、铁代谢指标的相关性。结果高荧光强度网织红细胞(HFR)在铁缺乏各期,均显著高于正常组(P<0.05);MFR和网织红细胞成熟(RM)指数在红细胞生成缺铁期和缺铁性贫血期,均显著高于正常组(P<0.01);sTfR在铁耗竭期,明显升高,与正常组有显著性差异(P<0.01)。sTfR和MFR、RM指数呈明显正相关;但同血红蛋白呈负相关。结论 HFR、MFR、sTfR及RM指数可作为早期铁缺乏的敏感指标。 Objective To investigate the relationship between the parameter of reticulocyte and serum transferrin receptor(sTfR) in the female adolescents of iron deficiency,and to assess the early sensitive index of iron deficiency.Methods A total of 279 females,aged 12-15 years,who were investigated for the measurement of serum ferritin,hemoglobin and serum iron,were divided into four groups.The parameter of reticulocyte and sTfR were assayed.Data were compared with each other in different groups.Survey the relationship between the sTfR and parameter of reticulocyte,iron parameters.Results The average high fluorescence reticulocytes(HFR) in iron depletion phase,iron-deficient erythropoiesis and iron-deficiency anemia were significantly higher than for the healthy subjects(P0.05,P0.01,P0.01,respectively).The average middle FR(MFR) and reticulocytes maturity index(RMI) were no significant difference between iron depletion phase and healthy control.While,in iron-deficient erythropoiesis and iron-deficiency anemia,they were significantly higher than the healthy subjects(P0.01).There were no significant differences of the ret(%) between the different group.The sTfR in iron depletion phase was significantly higher than the healthy subjects(P 0.01).Its concentrations correlated significantly with MFR,RMI,and hemoglobin levels.Coclusion HFR,MFR,sTfR and RMI are as early sensitive indexs of iron deficiency in female adolescents.
