胫骨平台骨折的内固定治疗

Treatment of Tibial Plateau Fracture with Internal Fixation

目的观察胫骨平台骨折内固定治疗的临床疗效。方法选择2008年2月～2011年6月的胫骨平台骨折患者63例,按Schatzker分为6种类型,其中Ⅰ型18例,Ⅱ型15例,Ⅲ型9例,Ⅳ型11例,Ⅴ型6例,Ⅵ型4例,不同类型的骨折患者分别采用不同的手术方案进行治疗。分别采用LKSS评分、SF-36量表评分进行术前、术后统计和比较,并用Hohl评分标准进行膝关节功能恢复评价。结果 63例患者均获得随访,随访期6～15个月,平均12.6个月。术后LKSS评分及SF-36量表评分均优于术前(P<0.05)。采用Hohl评分标准评估膝关节功能,恢复良好,优良率92.06%。结论内固定治疗胫骨平台骨折,术后膝关节准确复位,内固定坚强,功能恢复良好且明显改善生活质量。

Objective To observe the clinical effect of the treatment of tibial plateau fracture with internal fixation. Methods All 63 patients with tibial plateau fracture from February 2008 to June 2011 were divided into six types according to Schatzker, of which 18 cases were of type Ⅰ , 15 were of type Ⅱ , 9 were of type Ⅲ, 11 were of type Ⅳ, 6 were of type V and 4 were of type Ⅵ . Different types of fractures in patients were treated with different Surgical options. LKSS score and SF-36 scale scores were compared before and after the operation respectively, and evaluted knee recovery function by using Hohl score. Results All 63 patients were followed up for an average of 12.6 months （ range 6-15 months ）. LKSS score and SF-36 scale scores after operation were better than those before operation （ P 〈 0.05 ）. Knee function was recovered well by using Hohl score standards with the good rate of 92.06%. Conclusion After the treatment of tibial plateau fracture with internal fixation, the reduction of knee joints have been treated accurately and firmly. The function resumes well and the quality of life has also improved obviously.
