摘要
目的评价依达拉奉对单肺通气患者肺组织氧化应激反应的影响。方法择期拟行食管癌根治术患者30例,性别不限,ASA分级I或Ⅱ级,年龄40~64岁,体重50—85kg,采用随机数字表法,将其随机分为2组(n=15)。依达拉奉组(E组)气管插管后即刻,以60.0mg/h速率静脉输注依达拉奉30min,随后以7.5mg/h速率输注至术毕。对照组(C组)给予等容量生理盐水。分别于切皮前即刻、单肺通气30min、双肺通气30min时采集动脉血样和静脉血样,进行动脉血气分析,计算氧合指数、肺泡一动脉血氧分压差以及呼吸指数,并测定静脉血血清丙二醛(MDA)和8-异前列腺素F2α的浓度。结果与C组比较,E组双肺通气30min时肺泡-动脉血氧分压差、呼吸指数、血清MDA和8-异前列腺素F2。浓度降低(P〈0.05),氧合指数差异无统计学意义(P〉0.05)。结论依达拉奉减轻单肺通气患者肺损伤的机制与抑制肺组织氧化应激反应有关。
Objective To investigate the effects of edaravone on oxidative stress response during one-lung ventilation (OLV). Methods Thirty ASA Ⅰor Ⅱ patients of both sexes aged 40-64 yr, weighing 50-85 kg undergoing elective esophageal cancer resection were randomly divided into 2 groups ( n = 15 each) : control group (group C) and edaravone group (group E). In group E edavavone was infused immediately after tracheal intubation at 60.0 mg/h for 30 min. Then the infusion rate was reduced to 7.5 mg/h and maintained until the end of operation. In group C equal volume of normal saline was infused instead of edaravone. Arterial and venous blood samples were taken immediately before skin incision,at 30 rain of OLV and 30 rain after re-expansion of the collapsed lung for blood gas analysis and determination of serum MDA and 8-iso-PGF2α concentrations. PaO2/FiO2, PA-α DO2 and respiratory index (RI) (RI = PA-α DO2/PaO2 ) were calculated. Results PA-α DO2, RI and serum concentrations of MDA and 8-iso-PGF2α were significantly lower at 30 min after re-expansion of the collapsed lung in group E than in group C( P 〈 0.05). Conclusion Edaravone can attenuate oxidative stress response during OLV.
