摘要
目的：探讨胫骨平台骨折的临床治疗方法。方法：对桐庐县第一人民医院1993年8月—2009年12月间287例胫骨平台骨折病例进行回顾性调查分析,病例男183例,女104例;年龄19-65岁,平均年龄42.8岁;闭合性损伤193例,开放性损伤94例,按照Sehatzker分型,I型53例,型46例,型61例,型63例,V型47例,型17例;合并膝关节韧带、半月板损伤36例,其中交叉韧带损伤23例,半月板损伤32例,侧副韧带损伤53例。保守治疗84例,手术治疗203例。结果：该组病例全部获得随访,随访时间6-36个月,平均18个月。骨折全部骨性愈合。无内固定断裂及骨不连发生。按Bostman等的临床评分标准,对功能、疼痛程度、步态、膝关节稳定程度和功能活动范围5个项目进行综合评估,优184例,良74例,中18例,差11例。总治疗优良率89.9%。结论：胫骨平台骨折的治疗是一个极富挑战性的课题,笔者认为通过合理的骨折固定技术使骨折达到可靠的愈合,再配合适当的功能锻炼,是胫骨平台骨折治疗成功的关键。
ObjectiveTo explore the ways to treat fracture with tibial plateau.Methods Clinical data of 287 patients with fracture of tibial plateau were retrospectively analyzed.Results All patients were followed up from 6 to 36 months,with an average time of 18 months.All patients were cured with bony union.Integration assessment by clinical scale of marks indicated that 184 patients were cured excellent,74 good,18 common,and 11 bad.89.9% patients were excellent and good.Conclusion The treatment of fracture with tibial plateau is challenging.The key treatment of fracture with tibial plateau is reasonable fracture fixation technique.
