腹腔镜与开腹手术对结直肠癌患者术后恢复的影响

The comparison of effects of laparoscopic and open operation on colorectal cancer postoperative recover

目的对结直肠癌根治术患者采用腹腔镜与传统手术方法进行比较,观察患者术后恢复效果。方法将75例结直肠癌患者按自愿原则分为腹腔镜手术组(腹腔镜组)32例和传统手术组(传统组)43例,比较2组患者术中出血量、手术时间、术后疼痛程度、术后恢复情况及医护患满意率等。结果腹腔镜组患者的术中出血量和住院天数等少于传统组(P<0.01);术前、术后血红蛋白无显著性差异(P>0.05);自理能力和肠功能恢复优于传统组(P<0.05);疼痛反应轻于传统组(P<0.05);满意率高于传统组(P<0.05)。结论腹腔镜组在术中出血、术后疼痛反应、肠道功能恢复及住院天数等方面都优于传统组;自我护理有助于调动和发挥患者的主观能动性,促进术后肠道功能的恢复;患者术后疼痛反应与手术方式相关,微创手术是今后手术的方向,正确的宣传教育可帮助患者克服强烈的心理应激反应,提升护理服务满意度。

Objective To investigate the effects of laparoscopic and open operation on colorectal cancer postoperative recover. Methods A total of 75 consecutive patients with colorectal diseases were volunteered to laparoscopic surgery（n=32） or open operation（n=43）. The data of operative time,Blood loss,postoperative ache,recovery of bowel function,satisfied rate in the two groups were collected and analyzed. Results The Blood loss,postoperative ache,recovery of bowel function in the laparoscopie group was significantly lower than that in the open group （P〈0.05）. The patients in the laparoscopic group had a better satisfied rate （P〈0.05）. Conclusions Lapaoseope in radical resection for colorectal cancer posses less trauma,rapid postoperative recovery and better cosmetic results. It has better effect than traditional operation. Health education can help the patients to overcome the strong mental response. Interference nursing care can attain better satisfied rate and improve the patients＇ quality of life.
