胃十二指肠溃疡穿孔的外科治疗研究

目的探讨胃十二指肠溃疡穿孔的外科治疗研究。方法选取2009年1月～2011年1月收治胃十二指肠溃疡穿孔患者共100例,随机分为实验组与对照组。其中实验组患者采用穿孔修补加高选择性迷走神经切断术,而对照组患者采用常规方法进行开腹胃十二指肠溃疡穿孔单纯修补术治疗。结果两组患者的治愈率差异无统计学意义(P>0.05);两组患者的术中出血、肠蠕动恢复时间、术后止痛针剂使用次数、术后住院时间等方面比较,差异均具有统计学意义(P<0.05);两组患者出现伤口裂开、伤口感染及肠梗阻的发生率比较,差异均具有统计学意义(P<0.05),实验组出现不良反应的发生率均较低。结论穿孔修补加高选择性迷走神经切断术同时解决了穿孔和溃疡2个问题,可免除再次手术。

Objective To learn the surgery of perforation of gastro-duodenal ulcer.Methods 100 patients of perforation of gastro-duodenal ulcer from Jan 2009 to 2011 divided into 2 groups.The experiment group accepts perforation neoplasty and high selectivity gastric neurectomy.The control accepts common laparotomy perforation of gastro-duodenal ulcer single neoplasty.Results The diverse of cure rate of 2 groups has no significance（P〉0.05） .The diverse of procedrual bleeding,enterocinesia recovery time,analgesia frequency after surgery and length of stay of 2 groups has significance（P〈0.05） .The diverse of incidence of wound dehiscence,infection and ileac passion of 2 groups has significance（P0.05） .Conclusion The perforation neoplasty and high selectivity gastric neurectomy can resolve perforation and ulcer can save another operation.
