PPH加内括约肌切开术治疗混合痔伴肛裂26例

Therapeutic Efficacy of PPH plus Incision of Inner Sphincter in the Treatment of Mixed Hemorrhoids Complicated with Anal Fissure of 26 Cases

为探讨痔上黏膜环切术（PPH）加内括约肌切开术治疗混合痔伴肛裂的疗效，将混合痔伴肛裂的52例患者随机分为实验组（PPH加内括约肌切开术）和对照组（外剥内扎术加肛裂切扩术），每组26例，比较两组患者手术时间、住院时间及恢复工作时间、术后疼痛等指标，并进行统计分析。结果显示，实验组在手术时间、住院时间、恢复工作时间及术后疼痛等方面均优于对照组（P〈0．05，P〈0．01）。结果表明，PPH加内括约肌切开术治疗混合痔伴肛裂患者安全有效，术后疼痛轻，并发症少，有利于扩大PPH的治疗范围。

The objective of this study was to investigate the curative effect of PPH plus incision of inner sphincter for mixed hemorrhoids complicated with anal fissure. 52 patients with this diseases were randomized into two groups,patients in the study group（ n =26） underwent PPH plus incision of inner sphincter, while patients in the control group（ n =26） underwent external dissection ＆ internal ligation plus incision dilatation. As results,the aspects of operative time,hospital stay, time of resumption to work and reduction of postoperative pain were better in the study group than in the control group（ P 〈0.05, P〈0.01）. It is concluded that PPH plus incision of inner sphincter is safe and effective,and can be of less postoepratire pain and less complication in the treatment of mixed hemorrhoids complicated with anal fissure,is conducive to the expansion of PPH therapeutic range.
