摘要
目的探讨基质金属蛋白酶(MMP)-2和MMP-9基因多态性位点与脑卒中的关系。方法选择脑卒中患者348例为脑卒中组和健康体检者235例为对照组,采用限制性片段长度多态性分析技术检测MMP-2基因C1306T、C735T和MMP-9基因C1562T多态的分布特点并进行分析。根据诊断又将脑卒中患者分为脑出血组(116例)、脑梗死组(115例)、脑栓塞组(31例)、腔隙性脑梗死组(86例)。结果与对照组比较,脑卒中组收缩压、舒张压和TG水平明显升高,吸烟、颈动脉粥样硬化比例明显升高,差异有统计学意义(P<0.05)。脑出血组和脑梗死组MMP-9 C1562T的CT+TT基因型频率和T等位基因频率明显高于对照组(P<0.05)。脑梗死组MMP-2C735T的CC基因型频率和C等位基因频率明显高于对照组(P<0.05)。MMP-2、MMP-9各基因型均不是脑卒中预后影响因素。结论 MMP-2 C735T的C等位基因、MMP-9 C1562T的T等位基因是脑梗死的遗传易感基因之一,同时后者也是脑出血的遗传易感基因之一。MMP-2、MMP-9基因多态性与脑卒中预后无关。
Objective To investigate the relation of gene polymorphisms of matrix metalloprotein- ase-2 and matrix metalloproteinase-9 to stroke. Methods 348 patients with stroke(stroke group) and 235 healthy controls(control group) were recruited. PCR-RFLP was used to detect C1306T, C735T polymorphism of MMP-2 gene and C1562T polymorphism of MMP-9 gene. The stroke pa- tients were divided into cerebral hemorrhage group, cerebral infarction group, cerebral embolism group and lacunar infarction group according to diagnosis. Results Levels of systolic pressure, di- astolic pressure, triglyceride and proportion of history of smoking, carotid atherosclerosis were ob- viously higher in stroke group than in controls(P〈0.05). The CT and TT genotype frequencies and T allele of C1562T of MMP-9 gene were higher in cerebral hemorrhage and cerebral infarction groups than in controls(P〈0.05). The CC genotype frequency and C allele of C735T of MMP-2 gene were higher in cerebral infarction group than in controls(P〈0.05). Different genotypes of MMP-2 and MMP-9 were not influential factor of prognosis of stroke. Conclusions These results suggest that the C allele of C735T of MMP-2 gene and T allele of C1562T of MMP-9 gene are one of genetic susceptible genes to cerebral infarction, the later is one of genetic susceptible genes to cerebral hemorrhagic. Polymorphisms of matrix metalloproteinase-2 and -9 genes have no relation with prognosis of stroke.
