胆管癌组织中Survivin和Caspase-3蛋白表达及与预后的关系 被引量:2

摘要 目的探讨Survivin和Caspase-3蛋白在胆管癌组织中的表达及其临床意义。方法采用免疫组织化学方法检测57份胆管癌组织和23份胆管癌癌旁正常胆管组织中Survivin和Caspase-3蛋白的表达情况,分析Survivin和Caspase-3蛋白表达与临床特征及生存期的关系。结果胆管癌组织中Survivin蛋白表达明显高于、Caspase-3蛋白表达明显低于癌旁正常胆管组织(P均<0.05)。Survivin和Caspase-3蛋白的表达与胆管癌分化程度、TNM分期和淋巴结转移均有关(P均<0.05);胆管癌组织中Survivin和Caspase-3蛋白的表达呈负相关(P=0.001r,=-0.415);Kaplan-Meier生存分析表明,Survivin蛋白阳性者的总体生存时间短于阴性者,而Caspase-3蛋白阳性者明显长于阴性者(均P<0.05);COX多因素回归分析显示Survivin蛋白表达是胆管癌患者预后的独立因素(P<0.05)。结论 Survivin蛋白的高表达和Caspase-3蛋白的低表达对胆管癌的发生可能存在协同作用。 Objective To explore the expression of Survivin and Caspase-3 protein and their clinicopathologieal signicance in patients with cholangiocarcinoma. Methods The immunohistochemical method was used to analyze the Survivin and Caspase-3 protein expression in 57 cases of cholangioearcinoma and 23 cases of adjacent noncancerous normal bile duct tissues. The correlation between the expression of Survivin and Caspase-3 protein with the clinicopathological features and prognosis were analyzed. Results The Survivin expression in cholangiocareinoma was significantly higher than that in adjacent noncancerous normal bile duct tissues, but the Caspase-3 protein expression was on the contrary ( both P 〈 0.05 ) ; the expression of Survivin and Caspase-3 protein were related with differentiation grade, TNM stage and lymphnode metastas (both P 〈 0.05); there was inverse correlation bewteen the expression of Survivin and Caspase-3 protein (P =0. 001, r = -0. 415) ; The kaplan-Meier analysis showed that the overall survival time of patients positively expressed Survivin was significantly shorter than that of patients negatively expressed Survivin, but the Caspase-3 protein expression was on the contrary ( both P 〈 0.05 ) ; by Cox regression analysis, the Survivin expression was an independent prognostic factor( P 〈 O. 05). Conclusions The high expression of Survivin and low expression of Caspase-3 protein may be a synergistic effect in the progression of cholangiocarcinoma.
