早中期慢性肾脏病患者大动脉僵硬及其影响因素临床分析

Clinical analysis of influential factors on arterial stiffness in early metaphase chronic kidney disease patients

摘要 目的探讨早中期慢性肾脏病(CKD)患者大动脉僵硬的临床特征及其影响因素。方法收集新疆医科大学第一附属医院204例CKD1~4期患者颈-股动脉脉搏波速度(CF-PWV)、生化指标等临床资料并进行相关分析。结果 CKD患者CF-PWV正常组和异常组之间在年龄、肾小球滤过率、收缩压、血尿酸两组间的差异有统计学意义(P<0.05);单因素相关分析显示,CF-PWV与年龄、收缩压、尿酸、空腹血糖、纤维蛋白原水平呈正相关,而与肾小球滤过率呈负相关;多因素逐步回归提示CF-PWV与CKD分期、血尿酸分级、年龄分级独立相关。结论肾小球滤过率、血尿酸、年龄是早中期CKD患者发生动脉粥样硬化的独立危险因素。 Objective To explore the influential factors of arterial stiffness inpatients with 1~4 phage chronic kidney disease.Methods A retrospective study was conducted with 204 chronic kidney disease patients in 1~4 phage treated in the First Affilated Hospital of Xinjiang Medical University and the influential factors of arterial stiffness were then analyzed.Results There were significantly difference in age,eGFR,systolic blood pressure(SBP),uric acid(UA) between pulse wave velocity(CF-PWV) abnormal group and CF-PWV abnormal group(P0.05);correlation analysis showed that arterial stiffness had positive correlation with age,SBP,UA,fasting plasma glucose(FPG),FIG and negative correlation with eGFR;multiple logistic regression analysis showed that arterial stiffness was positively and independently associated with CKD,UA and age.Conclusion eGFR,UA and age are the independent risk factors of arterial stiffness in early metaphase CKD patients.
