美学区单牙种植体周软组织美学疗效及患者主观满意度研究 被引量:18

The study of peri-implant soft-tissue alterations and patient satisfaction in single-tooth implant in the esthetic zone

摘要 目的:种植体周软组织美学疗效是种植治疗成功与否的主要因素之一,目前尚缺乏一种有效的方法用于评估种植体周软组织的远期美学变化。本研究通过观察美学区种植体周软组织变化,分析口内照片能否作为美学区种植体周软组织相关参数的收集方法。方法:收集28例美学区单个种植体修复病例,按照一定标准拍摄基线和随访时种植体周软组织照片,两位观察者用红色美学指数,(t he pi nk est het i c score,PES)评估口内照片各自得分,对比基线以及随访时PES值变化。以视觉模拟评分法(vi sual anal ogue scal e,VAS)作为指标测量患者的主观满意度,分析患者主观满意度与PES值之间的关系。结果:基线与随访时PES平均值为(8.68±2.69)和(10.37±2.13)(p<0.01)。VAS值为72.5~100,患者VAS与PES值存在相关性,回归系数具有统计学意义。结论:口内照片可作为收集种植体周软组织PES参数的方法。美学区单个种植体周软组织美学疗效在种植体牙冠戴入3个月明显改善,PES值可大体上反映患者对种植体周软组织美学效果的主观满意度。 Objective:The esthetic result of peri-implant soft tissue is a crucial factor in the success of implant therapy.However,no effective objective method exists for observation of this factor over the long term.The aim of this clinical trial was to determine whether oral photographs can be used for peri-implant soft-tissue data collection by observing peri-implant soft-tissue alterations after a single-tooth implant in the esthetic zone and to determine whether the pink esthetic score(PES) reflects degree of patient satisfaction with the peri-implant soft-tissue esthetic result.Methods:Twenty-eight consecutive patients underwent single-tooth implantation in the esthetic zone.Oral photographs were taken to collect data on the peri-implant soft tissue at baseline(implant placement) and at 3-month follow-up.Two observers assigned PES values to the peri-implant soft tissue in the photographs.Changes in PES value from baseline to 3-month follow-up were calculated.Degree of patient satisfaction was measured using a visual analogue scale(VAS) at 3-month follow-up.The correlation between the VAS and PES scores was calculated.Results:The mean ± SD PES value was 8.68 ± 2.69 at baseline and 10.37 ± 2.13 at follow-up(P .01).VAS values ranged from 72.5 to 100.A significant correlation was found between VAS and PES values.The linear regression of patient satisfaction and PES values was significant.Conclusions:Oral photographs can be used to collect PES data on peri-implant soft tissue.The esthetic result of peri-implant soft tissue is markedly improved 3 months after restoration of the PFM crown.The PES value generally reflects the degree of patient satisfaction with the peri-implant soft tissue esthetic result.
