CT肺动脉血管成像体质量相关个体化管电流-时间的设置 被引量:3

摘要 【目的】探讨多层螺旋CT肺动脉血管成像(MDCTPA)根据体质量个体化调节管电流的可行性及体质量相关的最佳管电流-时间设置。【方法】90例临床怀疑肺动脉栓塞(PE)的患者接受了MDCTPA检查,前瞻性、随机分为6组,每组15例。比较各组图像质量主观评分、客观评价及有效吸收剂量。【结果】各组图像主观评价差异有统计学意义(P=0.008)。各组患者的有效吸收剂量差异有统计学意义(P<0.01)。C组图像与F组图像比较,图像客观评价与主观评分差异均没有统计学意义,而C组有效吸收剂量均值较F组降低47.8%。【结论】MDCTPA根据体质量个体化调节管电流-时间是可行且有效的,取"1.0mA.s/kg"作为临床指引进行个体化管电流-时间调节,能很好地平衡图像质量与有效剂量的关系。 [Objective] To assess the feasibility of individual-adapted tube current-time scanning protocol for multiple detector row computed tomography pulmonary angiography(MDCTPA).[Methods] Ninety patients with suspected pulmonary embolism(PE) accepted MDCTPA examination.All patients were randomized divided into 6 groups(15 each).Objective and subjective image quality and effective dose(ED) were assessed and compared in these groups.[Results] The differences of subjective image quality scores and Effective dose(ED) in these 6 groups were significantly different(P = 0.008,P 0.01,respectively).There were no significant differences between group C and group F in objective/subjective parameters.The mean value of ED in group C was reduced by 47.8% than group F.[Conclusion] An individually adapted scanning protocol of MDCTPA using tube current-time based on patient's body weight could be effectively performed in clinical setting.With the tube current-time of 1.0 mA·s/kg,balance between the image quality and reduction of radiation exposure could be achieved.
