前房异物165例临床分析

THE ANALYSIS OF FOREIGN BODIES IN THE ANTERIOR CHAMBER IN 165 CASES

本文分析厂我院1966—1988年间收治的前房异物165例166眼,左眼多于右眼,年龄在7—40岁者占83.6%,非金属异物109例,金属异物55例,性质不明3例,以石头和铁异物居多,异物多位于虹膜面(97例)和前房角(40例),术前盲目率27.8%,术后盲目率16.3%,术后视力在0.05—0.2 27眼。0.3—0.9 66眼。>1.0 43眼,<0.05 30眼。本文重点讨论了前房异物的处理,术前应缩瞳,降低眼压,根据异物的大小、位置和有无包裹选择手术,如行联合手术,应先摘除异物后作并发症手术。最好采用显微手术,可提高成功率,减少手术并发症。

One hundred and sixty-six eyes of 165 patients with foreign bodies in the anterior chamber who were treated at our center from 1966 to 1988 were retrospectively reviewed.Among them there were 109 eyes with nonmetal foreign bodies, 55 eyes with metal foreign bodies and three eyes with foreign bodies of unknown nature.Most of these foreign bodies were stone or iron.83.6% of the pati- ents were in the age range from 7 to 40 years,and the left eyes were affected mole than the right eyes. Mostly,the foreign bodies were located on the sur- face of iris (97 cases) and at the anterior chamber angle (40 cases).The rates of blindness were 27.8% and 16.3% before and after operation.The postope- rative visual acuity were:0.05—0.2 in 27 eyes,0.3— 0.9 in 66 eyes,>1.0 in 43 eyes and the rest of the eyes lower than 0.05.The presurgery treatment such as miosis and reduction of intraocular pressure,the selection of operative methods according to the na- ture,size,location of the foreign bodies and tissue wrapped up were discussed.If combined procedures should be done,it is better to remove the foreign bo- dies first and then perform the operation for compli- cations.
