造影剂与造影剂肾病关系的研究进展 被引量:18

摘要 随着现代医学发展,造影剂(CM)涉及的诊断及治疗手段逐渐增加,其在医学领域的价值不可替代,但CM导致的肾脏损害问题亦日渐凸显出来,造影剂肾病(CIN)是目前医院内发生急性肾衰竭的第3位常见原因,也导致晚期心血管事件发生率增加,虽发生率不足2%,但其发生后患者的病死率却高达34%。CIN的危险因素是肾髓质缺血,另外细胞凋亡、氧自由基损害、免疫因素、肾小管阻塞和CM对肾小管的毒性作用均以不同方式参与了CIN的病理生理过程。在高血压、动脉粥样硬化等人群中,由于内皮细胞功能损害这一共同的病理改变使其对CM的反应更加敏锐,更易造成CIN。本文就造影剂与造影剂肾病相关机制的研究作一综述。
