145例经宫腔镜诊断和治疗子宫内膜息肉临床分析

Hysteroscopic diagnosis and treatment of 145 cases with endometrial polyps

目的评估宫腔镜对子宫内膜息肉的诊断及治疗价值。方法总结145例因异常子宫出血经宫腔镜检查诊断为子宫内膜息肉的患者行宫腔镜下子宫内膜息肉电切术,术后全部进行病理检查。结果 145例患者中130例与病理符合,符合率为89.6%,1例合并鳞状细胞癌,4例病理证实为子宫内膜轻度非典型增生。结论宫腔镜结合病理检查可提高子宫内膜息肉的诊断率,宫腔镜手术治疗子宫内膜息肉疗效确切,复发率低,对子宫内膜癌倾向及早期子宫内膜癌可及时发现并指导治疗。

Objective To evaluate the value of hysteroscopy in the diagnosis and therapy of endometrial polyps. Methods Between June 2009 and June 2010, 145 patients due to abnormal uterine bleeding with the diagnosis of endometrial polyps by the hysteroscope were included. Hysteroscopic resection were performed and, pathological examination after resection were carried out. Results The diagnosis of 130 cases were based on the pathology, the consensus rate was 89.6%. One patient was complicated with squamous cell carcinoma and 4 patients were confirmed to have low-grade atypical endometrial hyperplasia. Conclusions Hysteroscopy combined with pathology examination can improve the accuracy of the diagnosis of endometrial polyps and trans-cervical resection of polyps is effective and with low recurrence rate. Trans-cervical resection of polyps can provide evidence for timely discovery and therapy of atypical endometrial hyperplasia and endometrial carcinoma.
