摘要
目的观察Survivin与nm23-H1蛋白在大肠癌及其癌旁组织中的表达情况,探讨其联合检测与淋巴结转移的关系。方法采用免疫组化S-P法研究66例大肠癌和15例癌旁组织的Survivin和nm23-H1的表达情况,分析其与临床病理特征的关系。结果在大肠癌及其癌旁组织中,Survivin阳性率分别为75.7%、62.1%,有显著性差异(χ2=13.254,P=0.000);nm23-H1的阳性率分别为26.7%、93.3%,差异有统计学意义(χ2=4.125,P=0.042)。Survivin的表达与肿瘤分化程度、淋巴结转移、肝转移、Dukes分期有关(P<0.05)。nm23-H1的表达与淋巴结转移、肝转移、Dukes分期有关(P<0.01)。Survivin阳性率与淋巴结转移呈正相关(r=0.247,P=0.046),nm23-H1阳性率与淋巴结转移呈负相关(r=-0.359,P=0.003)。Survivin与nm23-H1蛋白表达呈负相关(r=-0.296,P=0.016)。结论 Survivin蛋白的抑制凋亡作用和nm23-H1蛋白的抑制转移作用可能在大肠癌的发生、发展过程中起重要作用,联合检测是判断大肠癌患者淋巴结转移倾向及预后的重要指标。
Objective To determine the relationship of the expression of Survivin and nm23-H1 protein with lymph node metastasis(LNM) in human colorectal carcinoma and cancer adjacent tissues.Methods The expression of Survivin and nm23-H1 protein was examined in 66 tissue samples of colorectal cancer and 15 cases of adjacent tissues using immunohistochemical S-P method,and the results were analyzed for its relationship with clinicopathological features.Results In colorectal carcinoma and adjacent tissues,Survivin positive rates were 75.7% and 62.1%,respectively,with significant difference(Chi sqaure=13.255,P=0.000);while those of nm23-H1 were 26.7% and 93.3% respectively,with statistically significance(Chi sqaure=4.125,P=0.042).Survivin protein expression was related with tumor differentiation,LNM,liver metastasis,Dukes stage(P0.05).The expression of nm23-H1 protein was related with LNM,liver metastasis,Dukes stage(P0.01).Positiveness to Survivin protein had positive correlation with the LNM(r=0.247,P=0.046),while that to nm23-H1 protein had negative correlation with the LNM(r=-0.359,P=0.003).Negative correlation was found between Survivin and nm23-H1 expression(r=-0.296,P=0.016).Conclusion The anti-apoptotic effect of Survivin protein and the anti-metastastic effect of nm23-H1 protein may play important role in the occurrence and development of colorectal carcinoma.It may be an important prognostic indicator to determine lymph node metastatic potential and prognosis in human colorectal carcinoma using joint detection of them.
