单侧椎弓根螺钉及单枚融合器治疗腰椎间盘突出症近期疗效观察 被引量:1

摘要 目的观察单枚椎间融合器和单侧椎弓根螺钉固定、椎间融合治疗腰椎间盘突出症的近期临床效果。方法本组35例,以单间隙发病为主及腰痛和单侧肢体为主要症状,术前动力位片提示不稳定,滑脱在Ⅰ度以内或骨质增生致侧隐窝狭窄明显的腰椎间盘突出症,年龄在45~71岁之间的患者,行半椎板开窗椎间盘切除椎间单枚Cage植入、单侧椎弓根内固定手术治疗。结果 34例获得随访,时间6~36个月,平均18个月。6个月后X线或CT提示椎间融合,椎间融合器无移位,无明显沉降,内固定无松动及断裂。疗效评定按日本骨科协会(JOA)腰椎疾病29分评分法进行疗效评定,优20例,良12例,可1例,差1例,优良率为94.1%,脊柱融合率为97.1%。结论单侧椎弓根螺钉固定及单枚椎间融合器治疗腰椎间盘突出症近期临床效果满意,是一种可行的脊柱外科技术。 Objective To bserve the fusion between vertebra single pieces and unilateral pedicle screws,between vertebra fusion treatment of lumbar disc herniation recent clinical effect.Method 35 patients,at the age of 45 to 71 years old,did half lamina window disc resection between vertebra single medal Cage implant,unilateral pedicle fixation operation.Results Six months after X-ray or CT tip between vertebra fusions,fusion between vertebras was no shift,without obvious settlement,without loosening and fracture fixation.Curative effect evaluation: optimal 20 cases good in 12 cases,can be in 1,1 case was poor,for 94.1%,spinal fusion rate was 97.1%.Conclusion Unilateral pedicle screws and single pieces of the fusion between treatment prolapse of lumbar intervertebral disc achieved recent clinical effect satisfaction and is a feasible spinal surgery.
