丙泊酚联合地塞米松对早期脓毒症大鼠炎症因子的影响

Effect of propofol combined with dexamethasone on the inflammatory factors of early sepsis in rats

目的观察丙泊酚联合地塞米松对早期脓毒症大鼠炎症因子的影响。方法采用大鼠尾静脉注射脂多糖复制脓毒症模型。按完全随机化原则分为5组:正常对照组(N)、脂多糖组(L)、脂多糖+地塞米松组(LD)、LPS+异丙酚组(LP组)、LPS+异丙酚+地塞米松组(LPD组)。各组大鼠于造模后麻醉处死,取腹主动脉血,用酶联免疫吸附法(ELISA)检测血清肿瘤坏死因子-α(TNF-α)、白细胞介素-1β(IL-1β)、白细胞介素-6(IL-6)和白细胞介素-10(IL-10)。结果 L组TNF-α、IL-1β和IL-6较对照组明显升高,IL-10则明显降低(P均<0.05);LD组、LP组、LDP组同L组比较,其结果显示TNF-α、IL-1β和IL-6均明显降低,IL-10明显升高(P<0.05;P<0.05;P<0.05);LDP组同LP组、LD组比较,LDP组TNF-α、IL-1β和IL-6表达量明显下降,IL-10明显升高(P<0.05)。结论 LPS可迅速引起TNF-α、IL-1β和IL-6的释放,抑制IL-10的表达;地塞米松、丙泊酚干预后,TNF-α、IL-1β和IL-6的量明显减少,IL-10明显升高,且两药联用干预效果更明显,对早期脓毒症大鼠有明显保护作用。

Objective To observe the effect of propofol combined with dexamethasone on the inflammatory factors of early sepsis in rats.Methods Rats were randomly assigned into group N,group L,group LD,group LP,and group LPD,which underwent intravenous injection through the tail vein with 10 ml 0.9% Saline,lipopolysaccharide（LPS）,LPS and dexamethasone,LPS and propofol,LPS and propofol plus dexamethasone,respectively.All the rats were killed through anesthesia,and their abdominal aortic blood were detected for the levels of serum tumor necrosis factor-α（TNF-α）,interleukin-1β（IL-1β）,interleukin-6（IL-6） and interleukin-10（IL-10） by enzyme-linked immunosorbent assay（ELISA）.Results In group L,the levels of TNF-α,IL-1β and IL-6 was significantly higher than that in group N,while the levels of IL-10 was significantly lower（P0.05）.Compared with group L,the levels of TNF-α,IL-1β and IL-6 were significantly decreased in group LD,group LP and group LDP,with levels of IL-10 significantly increased..Compared with the group LD and group LP,the levels of TNF-α,IL-1β and IL-6 were significantly decreased in group LDP,with IL-10 significantly increased.Conclusion LPS could induce the release of TNF-α,IL-1β and IL-6,while inhibit the expression of IL-10.After the intervention of dexamethasone and propofol,the levels of TNF-α,IL-1β and IL-6 were significantly reduced,with IL-10 significantly increased.The combination of dexamethasone and propofol had significant protective effect on rats in the early stage of sepsis.
