剖宫产术中行子宫肌瘤剔除术安全性探讨

Safety and feasibility of performing myomectomy during cesarean section

目的观察剖宫产术中行子宫肌瘤剔除术的安全性及可行性。方法回顾分析我院2006年1月至2008年1月收治的90例行剖宫产术中予行子宫肌瘤剔除术的患者资料,与同期90例单纯用剖宫产手术患者的临床疗效及其疾病预后进行对比分析。对两组患者平均手术时间、术中出血量、术后24h出血量、手术前后血红蛋白差值、平均住院时间及产褥病率进行比较。结果除剖宫产术中行子宫肌瘤剔除术的平均手术时间[(73.5±9.8)min]明显高于单纯剖宫产术的时间[(34.7±6.3)min](P<0.05)外,其他各项测量指标术中出血量、术后24h出血量、手术前后血红蛋白差值、住院时间及产褥病率指标差异均无统计学意义(P>0.05)。结论剖宫产术同时行子宫肌瘤剔除术切实可行,能避免二次手术或子宫切除等,临床上值得推广应用。

Objective To explore the safety and feasibility of myomectomy during cesarean section.Methods The clinical data of 90 patients（the study group） receiving myomectomy during cesarean section were retrospectively analyzed and compared with those of 90 patients（the control group） undergoing normal cesarean section from Jan.2006 to Jan.2008.Results The average operation time in the study group were（73.5±9.8） min,which is significantly longer than（34.7±6.3） min in the control group（P0.05）.There were no significant difference in the hemorrhage during operation and 24 hours after operation,the change of Hb before and after operation,the average length of hospital stay,and the pueperal morbidity between the two groups（P0.05）.Conclusion Myomectomy during cesarean section is safe and feasible,which is worthy to be extended in clinical practice.
