肛肠外科围手术期抗菌药物的使用情况分析

Analysis on Perioperative Use of Antimicrobial Agents in Anorectal Surgery

目的了解肛肠外科围手术期抗菌药物的使用情况。方法对2010年4月至7月3日手术患者出院归档病历177份进行统计分析。结果抗菌药物的预防性使用率100%,使用时间最长21 d,最短2.5 d,平均5.5 d;共计使用抗菌药物6大类12种,其中头孢菌素4种,青霉素类(口服)1种,硝咪唑类1种,氟喹诺酮类3种,氨基苷类2种,中草药制剂类1种;单用抗菌药物20.34%,二联及以上联用79.66%;在术前30 min至2 h内应用抗菌药物占25.42%。结论抗菌药物使用的随意性较大、起点较高,术前静脉用药执行较差,有待进一步加强管理。

Objective To understand the perioperative use status of antimicrobial agents in anorectal surgery.Methods The filing medical records in 177 surgical patients discharged from April to July 3,2010 were performed the statistical analysis.Results The preventive use rate of antimicrobial drugs was 100%.The longest use time was 21 d,the least 2.5 d,average 5.5 d.Total 6 categories and 12 varieties of antimicrobial agents were used,including 4 varieties of cephalosporins,1 variety of penicillins（oral）,1 variety of nitramisole,3 varieties of fluoroquinolones,2 varieties of aminoglycosides and 1 variety of Chinese herbal medicine.Single drug use accounted for 20.34%and combination use with two-drug or more accounted for 79.66%.The antimicrobial drugs use in preoperative 30 min to 2 h accounted for 25.42%.Conclusion Antimicrobial drug use in anorectal surgery has more randomness,higher starting point and poor implementation of preoperative intravenous drug use,which need to strengthen management further.
