摘要
目的:探讨16排螺旋CT血管成像在主动脉夹层(Aortic dissection,AD)的影像诊断应用价值。方法:22例AD患者均行16排螺旋CT胸腹联合平扫和增强扫描,采用多平面重组(MPR)、大密度投影(MIP)、容积再现(VR)及表面遮盖显示(SSD)等方法进行三维重建。结果:原始横轴位图像对病变信息的存储最为可靠;不同的重组方式所得图像有不同的显示特点;27例均很好地显示了主动脉全程及其分支,显示真、假腔22例(100%),显示内膜片(100%),内膜破口17例(77.3%),腔内血栓形成9例(41%)。结论:16层螺旋CT血管成像是诊断AD准确、快捷、有效的检查方法。
Objective:To investigate the application value of the 16-slice spiral CT angiography (SCTA) in the imaging diagnosis of aortic dissection.Methods:Preand post-contrast CT scanning with 16-slice spiral CT(SCT) of chest and abdomen were performed in 22 patients with aortic dissection. Post processing techniques including nmhi-planar reformation (MPR),maximum intensity projection(MIP), volume rendering (VR) and surface shaded display (SSD) were conducted for three-dimensional reconstruction.Results:The original axial images preserved the most reliable and fundamental information.The images obtained by different refornation and reconstruction techniques had different characteristics. The whole aorta and its branches in these 22 cases were displayed satisfactorily by 16-slice SCTA. Aortic dissection was assessed in 22 cases,with true and false lumen(22 cases, 100%),intimal flaps(22 cases, 100%),entry of intimal tear (17 cases,77.3%) and thrombosed false lumen(9 cases,41%).Conelusion: 16-slice SCT angiography can fully display the anatomic details of aortia,which is an accurate, faster and effective check method to diagnose aortic dissection.
