连续腹腔镜胆囊切除术无胆管损伤1624例临床探讨

Consecutive laparoscopic cholecystectomy without bile duct injury in 1624 cases of clinical study

目的总结我院腹腔镜胆囊切除术安全成功的要点。方法对我院2004年4月～2010年7月连续开展腹腔镜胆囊切除术1624例临床资料进行总结分析。结果 1624例中,中转18例,中转率1.11%;选择暂停3例,暂停率0.18%;成功率98.71%。术后继发胆总管结石2例,胆囊床下放置引流管205例。术后住院2～5d,平均4d。结论加强安全意识,严格掌握手术指征,术中正确操作,解剖关系明晰,适时中转手术,合理放置引流管,是保证LC成功的关键。

Objective Summary laparoscopic cholecystectomy in our hospital security key to success.Methods On our hospital from April 2004 to July 2010 consecutive laparoscopic cholecystectomy 1624 patients clinical data analysis.Results 1624 cases,Transit 18 cases,Transit rate 1.11%;select Pause 3 cases,Pause rate 0.18%;the success rate 98.71%.Postoperative patients with secondary common bile duct stones 2 cases,205 cases with drainage.Postoperative hospital stay 2～5 days,average 4 days.Conclusion Enhancing safety awareness,strict operative surgical indications,intraoperative correct operation,anatomical relationship clear,Appropriate time to change the surgical approach,reasonable drainage,These are the guaranteed key to laparoscopic cholecystectomy success.
