土豆片在静脉注射碘对比剂外渗中的应用研究

The application of Potato chips in the extravasation of iodine contrast agent through intravenous injection

目的：探讨应用新鲜土豆片在320排动态容积CT机进行CT增强检查时静脉注射碘对比剂外渗的疗效及安全性.方法：选取2008年11月至2011年6月,对72例在进行CT增强检查中发生碘对比剂外渗的患者,注射部位发生疼痛、肿胀的现象,将72例患者随机分为观察组和对照组.观察组采用新鲜土豆切成薄片,对碘对比剂外渗肿胀部位进行敷贴.观察组用新鲜土豆,清洗干净,不用削皮,用刀将土豆从中间切开两块,再从两块的直径最大的地方切成0.2cm-0.3cm的薄片,在注射部位肿胀处敷盖土豆薄片,让薄片能够紧贴在皮肤上.薄片覆盖范围应超出肿胀部位6cm以上的位置,用手掌轻轻按压,让土豆薄片更好地贴在皮肤上,并用保鲜膜包裹土豆薄片,待土豆薄片干后再更换.直到注射部位肿胀完全消肿,才停止敷贴.对照组用两块12层的纱布浸有50%硫酸镁对注射肿胀部位进行湿敷,范围超出肿胀部位6cm以上.纱布干后,再换浸有50%硫酸镁的纱布继续湿敷.直到肿胀部位完全消肿为止.比较两组患者肿胀部位消肿时间、效果的差异.结果：通过实验得出：观察组用新鲜土豆片敷贴6-8小时后肿胀部位完全消肿,患者疼痛消失;对照组用50%硫酸镁湿敷9-11小时后患者肿胀部位完全消肿.其中有一例患者肿胀部位出现水泡,皮肤破损,96小时后才完全愈合.结论：通过实验得出：新鲜土豆对碘对比剂外渗的疗效明显优于50%硫酸镁.新鲜土豆价格低廉、含有大量淀粉和维生素B,用于治疗碘对比剂外渗,具有高渗作用,能够缓解局部肿胀,消炎止痛,效果明显.

Objective：To investigate the efficacy and safety in the extravasation of iodine contrast agent through intravenous injecting fresh potato chips on the 320 row dynamic volume CT examination. Methods： From November 2008 to June 2011,72 patients who have the extravasation of iodine contrast agent when underwent the enhanced-CT scan were randomly divided into the control group and the observation group according to the pain and swelling in the injection site. The observation group were treated with fresh sliced potatoes,which cleaned,not peel and cutted into two,and then from the two largest local in diameter cutted into 0.2cm -0.3cm sheets, and applyingt the swelling injection site. The coverage should extend beyond the location of more than 6cm, pressing gently so that potato slice could attach to the skin better,then wrapped with plastic wrap potato flakes until potato flakes to be dry before replacing. Stop when the swelling disappeared. The control group were treated by with two 12-layer gauze soaked with 50 magnesium sulfate and the coverage,were also more than 6cm. and back to 50% magnesium sulfate When gauze got dry,. Until the swelling disappeared. Compare the time and the effect. Results：The o observation group costed 6-8 hours when the control group costde 9-11 hours,including one patient who appear blisters and broken skin was completly healed after 96 hours. Conclusion：The fresh potatoes is significantly better than 50% magnesium sulfate in treating iodine contrast agent extravasation. The fresh potatoes is low price, and rich in starch and vitamin B, which have an obvious effect on treating of iodine contrast agent extravasation for the hyperosmolar effect to alleviate the swelling and inflammatory pain.
