胸壁结核的诊断和外科治疗之探讨

The study of diagnosis and surgical treatment of the chest wall tuberculosis

目的总结探讨胸壁结核的临床特点和外科治疗。方法采用回顾分析法对2003年7月～2009年6月间收治的157例(其中复发性15例)胸壁结核患者的临床资料进行分析。结果所有患者均经过外科手术治愈。术后病理检查证实均为胸壁结核。术后随访10个月～6年无1例再发。结论胸壁结核的临床特点具有隐匿性、多形性,外科手术治疗效果好。胸壁结核病灶清除术为不定型手术,手术前后必须正规、有效、足疗程抗结核治疗,术中彻底清除病灶、消灭死腔及术后正确的管理,是手术成功,避免复发的关键。

Objective To investigate the clinical features of chest wall tuberculosis and surgical treatment of chest wall tuberculosis.Methods Clinical data of the 157 patients with chest wall tuberculosis who were treated（including 15 cases with recurrence） from July 2003 to June 2009 were retrospectively analyzed.Results All patients were cured after surgery.All patients were pathologically confirmed chest wall tuberculosis.All patients were followed up for 10 months to 6 years no case of recurrence.Conclusion The clinical features of chest wall tuberculosis is latent,polymorphism,and surgical treatment is the effective treatment.Chest wall tuberculosis by surgical operation is not atypical surgery-the lack of uniform standards,before and after surgery must be regular,effective and full course of anti-tuberculosis,in surgery completely removed lesions,eliminated potential lacunar and proper postoperative management,these are the key to successful operations and to avoid recurrence of tuberculosis lesions.
