乳腺癌保乳术后放疗瘤床加量野的研究 被引量:3

摘要 目的比较模拟机透视下依据银夹确定瘤床加量范围以及CT定位后通过勾画肿瘤靶区(GTV)确定范围的差异和优劣性,并探讨差异产生的原因。方法选取早期乳腺癌保乳术后患者5例的CT定位图像,在计划系统中勾画GTV、银夹和手术切口,分别设计经模拟机透视和CT定位下的瘤床加量野。结果 5例患者中,经模拟机透视和CT定位下的瘤床加量野其在(x,y)方向上的差距中,最大值为(±0.5,-1)cm,均值为(0.1,-0.5)cm;射野面积差距中,最大值为2.5cm2,均值为-0.1cm2。结论银夹和手术切口标记范围不能准确代表实际瘤床。模拟机透视下的瘤床加量野操作简单,直观明了,但可能会导致部分患者瘤床受照剂量不够或造成正常组织不必要受照。经CT定位确定乳腺癌瘤床加量野可以充分包括术腔,避免靶区遗漏,其不规则射野有效地保护了肺、心脏等正常组织,降低其受照量。
