2型糖尿病心室重构的临床研究

摘要 [目的]探讨糖尿病对心室重构的影响.[方法]选取2型糖尿病患者136例(DM组),健康对照组90例.检测其血压、身高(m)、体重(kg),血糖、血脂等指标.采用超声心动图(UCG)测量舒张期末左室内径(LVD),舒张期末左室后壁厚度(LVPW)、舒张期末室间隔厚度(IVS)以评估左室结构.[结果]DM组与对照组比较LVD、IVS、相对室壁厚度(RWT)差别无统计学意义(P〉0.05),DM组LVPW、左室重量(LVM)、左室重量指数(LVMI)高于对照组差别有统计学意义(P〈0.05).DM组136例左室肥厚发生数8例(5.88%),发生率高于对照组(χ2=5.177,P=0.025).[结论]糖尿病患者更易发生左室重构,且左室重构类型以向心性肥厚多见. [Objective] To explore the effect of diabetes mellitus(DM) on heart remodeling. [Methods]Totally 136 DM patients(DM group) and 90 healthy controls(control group) were selected. Blood pressure, height, weight, blood glucose and lipids were measured. Ultrasonic eardiography(UCG) was used to measure left ventricular end-diastolic diameter(LVD), left ventrieular posterior wall(LVPW) and interventrieular septal thickness (IVS) to evaluate left ventricle structure. [Results]There was no significant difference in LVD, IVS and R-wave threshold(RWT) between DM group and control group( P〉0.05). LVPW, left ventricle mass(LVM) and left ventricle mass index(LVMI) in DM group were higher than those in control group, and there was significant difference(P〈0.05). In DM group, the prevalence of left ventricle hypertropby(LVH) was 5. 88%(8/136) which was higher than that in control group (2 = 5. 177, P = 0. 025). [Conclusion] Left ventricle remodeling is more likely to occur in DM patients. Concentric hypertrophy is commonly seen. [Key words] diabetesmellitus,non-insulin-dependent; heart ventricle
