上海市闸北区性病门诊患者HSV－2感染率及其影响因素研究

Seroprevalence and correlates of herpes simplex virus type 2 infection among STD patients in Zhabei District,Shanghai

目的了解上海市闸北区性病门诊患者2型单纯疱疹病毒（herpessimplexvirustype2，HSV-2）感染率及其影响因素。方法抽取2008年7～8月间前往闸北区中心医院规范化性病门诊就诊的患者，进行问卷调查和HSV-2、HIV血清学抗体检测。结果共有256例就诊者接受调查，平均年龄（36．86±11．82）岁，大多数为男性（178人，占69．5％）、在婚（179人，占69．9％）、外地户籍（150人，占58．6％）、高中及以上文化程度（141人，占55．1％）。初次性行为年龄范围为13～39岁，平均年龄在（22．37±3．67）岁。222例（86．7％）调查对象目前有固定性伴，145例（56．6％）调查对象最近3个月有过商业性行为，7例（2．7％）调查对象有吸毒行为，1例（0．4％）HIV检测呈阳性。HSV-2感染率为30．1％，男性为25．8％，女性为39．7％。多因素Logistic回归分析显示，在控制了其他变量的混杂影响后，女性HSV-2感染率显著高于男性，差异有统计学意义（P=0．027）；最近3个月有商业性行为者HSV－2感染率高于无商业性行为者，差异有统计学意义（P=0．048）。结论上海市闸北区性病门诊患者中HSV－2感染率较高，其中女性、有商业性行为者是防治的重点人群。

Objective To determine the seroprevalence and correlates of herpes simplex virus type 2 （HSV-2） in- fection among sexually transmitted diseases （STD） patients in Zhabei District, Shanghai, Methods All patients attend- ing the STD clinic in Zhabei District Central Hospital, Shanghai from July to August in 2008 were interviewed with an anonymous questionnaire, and blood-tested for HSV-2 and HIV infections. Results A total of 256 patients attended this SIT） clinic and participated in the study. Of them, the average age was （36.86 ＋ 11.82） years, the majority were males （178/256,69.5%）, married （ 179/256, 69.9% ）, non-Shanghai residents （150/256,58.6%）, and educated at high school or above （ 141/256,55.1% ）. The age at first sex ranged from 13 to 39 years, with an average of （22.37 ＋ 3.67） years. 222（86.7% ） currently had fixed sex partners, 145（56.6% ） had commercial sex in the past three months, and 7 （2.7%） had used drugs. One person （0.4%） was tested positive for HIV. The HSV-2 seroprevalence was 30.1%, 25.8 % for males and 39.7 % for females. A multiple Logistic regression analysis indicated that, after adjustment for potential confounding effects, females and those having had commercial sex in the past three months were more likely to be infected with HSV-2. Conclusions HSV-2 infection was highly prevalent among STD patients in Zhabei District, Shanghai. Among them, females and those engaging in commercial sex should be most important for HSV-2 prevention and control.
