手术切除联合角膜缘干细胞移植及氟尿嘧啶治疗翼状胬肉临床观察

Clinical observation of surgical excision with limbal stem cell transplantation and fluorouracil in the treatment of pterygium

目的探讨手术切除联合角膜缘干细胞移植及氟尿嘧啶治疗翼状胬肉临床疗效。方法选择2006年10月至2009年3月收治的角膜缘翼状胬肉患者120例(均为单眼),随机分为3组,各40例,A组给予手术切除联合角膜缘干细胞移植及氟尿嘧啶治疗,B组给予手术切除胬肉+羊膜移植治疗,C组患者单纯给予翼状胬肉手术切除;观察3组治疗效果。结果 3组术后随访1～2年,平均(16.5±1.5)个月;A组患者术后不适症状轻时间短,角膜创面愈合快,术后治愈37例,复发3例,B组治愈32例,复发8例,C组治愈26例,复发14例,其中A组患者术后复发率明显低于其他两组(P<0.05)。结论手术切除联合角膜缘干细胞移植及氟尿嘧啶治疗翼状胬肉,术后复发率低,并发症少,愈合时间短,值得应用。

Objective To evaluate the clinical efficacy of surgical excision with limbal stem cell transplantation and fluorouracil in the treatment of pterygium.Methods 120 pterygium patients（all unilateral） in our hospital from October 2006 to March 2009 were randomly divided into 3 groups,each 40 cases,group A received surgical excision with limbal stem cell transplantation and fluorouracil;group B received surgery ＋ amniotic membrane transplantation;group C were given surgical excision only;observed treatment efficacy of three groups.Results The three groups were followed up for 1-2 years,average 16.5±1.5 months;patients in group A with mild symptoms in a short time,corneal wound healing quickly,37 patients cured after surgery,recurrence in 3 cases,32 patients of group B were cured and recurrence in 8 cases,group C were cured in 26 cases,14 cases of recurrence;the recurrence rate and complications of group A were significantly lower than the other two groups.Conclusion Surgical excision with limbal stem cell transplantation and fluorouracil for treatment of pterygium,recurrence rate and complications are fewer,healing time is short,and it is worthy of application.
