FOLFOX4方案治疗晚期胃癌的疗效观察

The observation of FOLFOX4 in the treatment of advanced gastric cancer

目的观察奥沙利铂(L-OHP)联合5氟尿嘧啶(5-Fu)、亚叶酸钙(CF)(FOLFOX4)方案治疗晚期胃癌的疗效及不良反应。方法全组27例,应用奥沙利铂85mg/m2静脉输注3 h,第1 d;亚叶酸钙200mg/m2静脉输注,第1、2 d;5-氟尿嘧啶400 mg/m2静脉推注,第1、2 d,5-氟尿嘧啶600 mg/m2静脉持续泵入22 h,第1、2 d,每2周重复,至少进行3周期。评估疗效、不良反应和总有效率,计算疾病进展时间(TTP)和总生存期(OS)。结果 27例患者总有效率50%,中位TTP 8个月(1～19个月),中位OS 9个月(3～20个月)。最常见的不良反应是外周感觉神经异常、消化道反应和骨髓抑制。但毒副反应可控。结论 FOLFOX4方案治疗晚期胃癌疗效好,不良反应可以耐受。

Objective To evaluate the efficacy and toxicity of oxaliplatin combined with 5-fluorouracil and leucovorin（FOLFOX4） in the treatment of advanced gastric cancer.Methods 27 patients with advanced gastric cancer were treated with chemotherapy of regiment oxaliplatin 85 mg/m2 ivgtt 3h d1;LV 200mg/m2 ivgtt 2 h followed by 5-Fu 400 mg/m2（bolus）and 5-Fu 600 mg/m2（22h-continuous infusion）d1,2 and repeated every 2 weeks,All patients received three cycles of chemotherapy at least.Results The total effective rate was 50%.The median time to progression（mTTP）was 8 months（1~19months）and the median overall survival（mOS）was 9 months（3~20months）.The major toxicities included peripheral sensory nerves toxicity,gastrointestinal reaction and bone marrow suppression.Conclusion FOLFOX4 is a high response regimen for advanced gastric cancer and its toxicity is tolerable.
