正常成人咀嚼肌表面肌电图的参数分析 被引量:9

Parameter analysis of surface electromyography recorded from masticatory muscles in adults with normal occlusion

摘要 目的:通过观察正常成人下颌不同运动状态时咀嚼肌表面肌电图(sEMG),探讨sEMG的不同参数分析反映咀嚼肌功能状态及运动形式上的差异性。方法:记录10例(男4例,女6例)正常成人双侧二腹肌前腹、咬肌、颞肌前腹、颞肌后腹,在下颌姿势位、前牙对刃、下颌最大前伸、双侧咀嚼、大张口、吞咽后紧咬牙等6种功能状态时的sEMG,分别进行时域和频域分析。结果:时域分析表明,除LDA在平均幅值分析时无显著性差异(P>0.05)外,咀嚼肌在行使不同动作时的平均肌电值、峰值、平均幅值和幅值均方根均有显著性差异(P<0.05或P<0.01);除姿势位和吞咽后紧咬牙时咀嚼肌的平均肌电值、峰值、平均幅值和幅值均方根,以及大张口时平均幅值和幅值均方根在咀嚼肌间无显著性差异(P>0.05)外,其他各个动作时咀嚼肌间均有显著性差异(P<0.05或P<0.01)。频域分析显示,咀嚼肌在行使不同动作时的平均功率频率和中位频率均有显著性差异(P<0.05或P<0.01);除平均功率频率在姿势位和下颌最大前伸时,以及中位频率在咀嚼和吞咽后紧咬牙时,咀嚼肌间无显著性差异(P>0.05)外,其他各个动作时咀嚼肌间均有显著性差异(P<0.05或P<0.01)。结论:sEMG的多参数分析能较好地反映不同下颌运动状态下各咀嚼肌的功能情况,提示临床上应对sEMG进行多参数综合分析才能做出全面的判断。 Objective:To observe the surface electromyography(sEMG) recorded from the masticatory muscles in separate jaw movement in adults with normal occlusion for the diversity by parameter analysis.Methods:The sEMGs were kept in 10 healthy adult volunteers(4 males,6 females) regarding bilateral anteroventral portion of digastric muscles,masseter muscle,anterior and posterior temporalis,movement states of postural position,edge-to-edge occlusion,jaw protraction,bilateral chewing,mouth opening and maximum clenching after swallowing,with parameter analysis of sEMG concerning time domain and frequency domain.Results:The time domain demonstrated significant differences in mean EMG(AEMG),spike analysis,mean amplitude(MA) and root mean square(RMS) of sEMG by mandibula movements recorded from each muscle and among muscles during each jaw movement except for MA seen in left digastric muscle(P0.05 or P0.01).No significant difference was found with AEMG,spike analysis,MA and RMS by postural position and clenching after swallowing as well as MA and RMS in state of maximally mouth opening(P0.05).The frequency domain indicated that mean power frequency(MPF) and median frequency(MF) were significantly different concerning sEMG in mandibula movements recorded from each muscle and muscles during jaw movements other than MPF in muscles by postural position and jaw protraction and MF in muscles by bilateral chewing and maximum clenching after swallowing(P0.05 or P0.01).Conclusion:Multi-parameter analysis of sEMG can exactly exhibit the function of masticatory muscles in separate jaw movements,suggesting that integral evaluation of sEMG parameters is necessary for mouth diseases.
