甘精胰岛素联合二甲双胍治疗2型糖尿病临床观察

Clinical observation of combined insulin glargine with metformin in type 2 diabetes mellitus

目的:观察甘精胰岛素联合二甲双胍对血糖控制不佳的超重或肥胖2型糖尿病患者的疗效。方法:对86例体重指数(BMI)≥24 kg/㎡,并且使用口服降糖药治疗后糖化血红蛋白(HbA1C)≥7.0%的2型糖尿病患者,在制定合理的饮食加运动治疗方案的基础上,采用甘精胰岛素加二甲双胍联合治疗。根据血糖水平调整甘精胰岛素及二甲双胍用量(以FBG达到<6.7 mmol/L,2 h BG<8.0 mmol/L为治疗目标),疗程6个月,比较联合治疗前后血糖、糖化血红蛋白、体重指数等指标变化。结果:治疗后与治疗前比较血糖、糖化血红蛋白均有明显好转(P<0.01),体重无明显增加,无严重低血糖事件。结论:此联合治疗可使超重或肥胖的2型糖尿病患者血糖、糖化血红蛋白得到理想控制,同时体重增加不明显。

Objective：To observe the effect of combined insulin glargine with metformin in treatment of type 2 diabetics,poor blood glucose control,with overweight or obesity.Methods： Eighty-six type 2 diabetics whose body mass index（BMI）≥24 kg/㎡ and glycosylated hemoglobin（HbA1c）7.0% received medication of insulin glargine combined with metformin on basis of intervention by diet and exercise.Dosage adjustment was in accordance with the level targeted at fasting blood glucose（FBG）6.7 mmol/L or 2 h BG8.0 mmol/L.After 6-month therapy,the changes of blood glucose,HbA1c and BMI were observed before and after medication.Results： The indicators,including blood glucose and HbA1c,improved a lot after therapy,with no incidence of rebound of body weight and hypoglycemia.Conclusion： Combined insulin glargine with metformin can be effective in type 2 diabetics with overweight or obesity.
