结直肠疾病的微创治疗体会

The experiences of mini-invasive surgical treatment for colorectal diseases

目的:探讨腹腔镜手术治疗结直肠疾病的适应证、安全性和可行性。方法:回顾分析2007年1月至2010年8月为226例结直肠疾病患者施行腹腔镜手术的临床资料。结果:222例成功完成腹腔镜手术,4例中转开腹。手术时间平均(172±65)min,术中出血量平均(95±41)ml。无死亡病例。随访186例恶性肿瘤患者平均26个月,局部复发4例(2.2%),远处转移5例(2.7%),肿瘤相关死亡7例(3.8%)。结论:腹腔镜手术治疗结直肠疾病安全、可行。腹腔镜手术治疗结直肠癌可取得与开腹手术相同的根治效果。

Objective：To evaluate the indications, safety and feasibility of laparoscopic surgical treatment for colorectal diseases. Methods：The clinical data of 226 patients with colorectal diseases treated by laparoscopie surgery from Jan. 2007 to Aug. 2010 were analyzed retrospectively. Results ： Laparoscopic eolorectal operation was successfully performed in 222 cases, and the other 4 cases were converted to open operation. The mean operating time was （ 172 ± 65 ） rain ; the average blood loss was （95 ± 41 ） ml. There was no perioperative death in these cases. The mean follow-up of the 186 patients with malignant colorectal tumor was 26 months,the local recurrence rate,distant metastasis rate and tumor related mortality was 2.2% （n =4） ,2.7% （n =5） and 3.8% （n =7） ,respectively. Conclusions：Laparoscopic operation for colorectal diseases is safe and feasible. Laparoscopic operation is as effective as conventional open surgery in the treatment of colorectal cancer.
