单侧唇裂鼻畸形Ⅱ期整复术定量分析

Quantitative analysis to both preoperative and postoperative measurements of secondary repair of unilateral cleft lip nasal deformity

目的 探讨单侧唇裂鼻畸形Ⅱ期整复术的手术方法及其客观定量分析.方法 回顾性分析自1999年1月至2009年1月收治的78例单侧唇裂鼻畸形患者.本组患者均采用改良Bardach法进行矫治,采用人体测量学方法制定测量指标,定点测量患者术前及术后随访照片,对鼻畸形特点及改良Bardach法的远期效果进行客观定量分析与评价.结果 本组共78例患者,术后随访2～4年,所有患者鼻外形恢复良好,鼻小柱居中,鼻基底抬高,双侧鼻孔基本等大,外形满意,瘢痕轻微.定量评价远期效果：优64例,良9例,可5例.结论 单侧唇裂鼻畸形Ⅱ期整复术前术后定量分析的方法可有效辅助手术方案的制定,针对不同患者制定个性化的治疗方案,取得满意的临床疗效,并且可以科学地评价术后远期效果.

Objective To explore the effect of secondary repair of unilateral cleft lip nasal deformity and quantitative analysis to the nasal characteristics. Methods A retrospective study was carried out on totally 78 patients with unilateral cleft lip nasal deformity received in our department from January 1999 to January 2009. A anthropometry method based on the quantitative analysis to the characteristics of nasal deformity was performed to evaluate and compare the preoperative and postoperative measure of the patients treated by modified Bardach＇s technique. Results After 2 to 4 years follow-up, all 78 cases received satisfactory results with centered columella nasi, isometric nostrils and slight scars. For the postoperative quantitative measurement results： excellent in 64 eases, good in 9 cases, and effective in 5 cases. Conclusion It could accessibly receive the optimal curative effect if the therapeutic schedule of the unilateral cleft lip nasal deformity is formulated over all situation based on quantitative analysis to the nasal characteristics. Also, it is a scientific approach to evaluate the outcome.
