全髋关节置换术临床路径的变异情况分析

Variance analysis of clinical pathway for patients with total hip arthroplasty

目的分析全髋关节置换术临床路径的变异情况,研究临床路径存在的问题并提出相应的改进措施。方法分析70例符合全髋关节置换术适应证的患者资料,并根据变异的标准及类型进行统计分析。结果 70例病例中有58例发生变异,包括15种变异情况,以负性变异、患者病情变化变异为主。结论全髋关节置换术临床路径需要完善运行流程,制订详细的纳入和排除标准,同时加强社区医院及康复医院的相应诊疗过渡,以更好地开展全髋关节置换术临床路径。

Objective To analyze the variance of clinical pathway applied in the patients with total hip arthroplasty, and then to find out the problems in the clinical pathway and then propose some improvement measures. Methods This research retrospectively collected the data of 70 cases who had the indication of the total hip arthroplasty, and analyzed variance according to the standards and types of variance. Results There were 58 variances in 70 cases, which were 15 types and mainly were negative variance. Most of variance came from patients and were uncontrollable variance. Conclusion We should improve the operation process, make detailed inclusive and exclusive criterion and enforce the transition of community hospitals and convalescent hospitals, so we can carry out total hip arthroplasty clinical pathway in a good way.
