壳聚糖明胶海绵对下颌阻生智齿拔除后生活质量的影响

THE INFLUENCE OF CHITOSAN SPONGE ON QUALITY OF LIFE AFTER EXTRACTING MANDIBLE IMPACTED WISDOM TOOTH

目的探讨壳聚糖明胶海绵对下颌阻生智齿拔除后生活质量的影响。方法选择拔除下颌阻生智齿316颗,随机分为3组。试验组111颗牙拔除后牙槽窝内植入壳聚糖明胶海绵;对照组102颗牙拔除后牙槽窝内植入碘仿明胶海绵;空白对照组103颗牙拔除后牙槽窝内不不放置任何填充物,分别咬棉球30min。采用改良的术后症状严重度量表(Postoperative symptom severity scale,PoSSe)综合量化评价患者拔牙术后的生活质量。结果试验组PoSSe分值为(19.58±16.63)分,对照组为(26.99±15.40)分,空白对照组为(22.81±12.52)分。试验组PoSSe评分明显低于对照组和空白对照组(P<0.05)。试验组和对照组牙龈点彩出现的例数比空白对照组大幅增加(P<0.05),口内出现血性分泌物例数明显少于空白对照组(P<0.05)。结论壳聚糖明胶海绵可促进拔牙创愈合,有效预防下颌阻生智齿拔除后干槽症的发生。

Objective To investigate the influence of chitosan sponge on quality of life after extracting mandible impacted wisdom teeth. Methods A total of 316 patients who were extracted mandible impacted wisdom teeth were selected and divided into 3 groups at random. In experimental group,the ehitosan sponges were embedded into teeth sockets after extracting, and the cotton ball were bited for 30rain ; In control group, the iodoform gelatin sponges were embedded into teeth sockets after extracting, and the cotton ball were bited for 30rain. Nothing was embedded into the teeth sockets after extracting in blank group. The improved postoperative symptom severity scale （PoSSe） was adopted to evaluate the quality of life after the operation. Results Experimental group scored （ 19.58 ± 16.63 ）, control group scored （26.99±15.40）,blank group scored （22.81± 12.52）. The value of three groups were analyzed by variance analysis, which led to significant difference as a whole. Conclusion Chitosan sponge can effectively accelerate the healing of wound and prevent dry socket after extracting mandible impacted wisdom teeth.
