后路侧前方减压椎体重建治疗胸腰椎爆裂骨折 被引量:6

摘要 目的探讨后路侧前方减压椎体重建椎弓根螺钉固定治疗重度胸腰椎爆裂骨折的疗效。方法2007年8月至2010年3月采用经后路侧前方减压椎体重建椎弓根螺钉固定治疗24例胸腰椎爆裂骨折患者,男19例,女5例;年龄24~53岁,平均38.3岁。经后路单侧行椎体次全切除、脊髓减压、钛网支撑植骨重建前中柱并行椎弓根钉内固定。采用美国脊柱损伤学会(ASIA)评分进行神经功能评估,通过X线片及CT片评估骨折复位、减压及骨融合情况。结果手术均顺利完成,平均手术时间为2.9h;术中平均出血量为1220mL。患者术后获6~24个月(平均14.3个月)随访。其中2例术后发生脑脊液漏。患者植骨均获融合,无内固定松动、断裂等并发症发生。固定节段椎体间生理曲度基本恢复正常,伤椎Cobb角矫正为15°~28°,平均21.7,椎管容积无明显丢失。患者神经功能均有不同程度的恢复。结论后路侧前方减压椎体重建短节段固定治疗重度胸腰椎爆裂骨折具有创伤小、安全性高、减压彻底及脊柱三柱即刻稳定等优点,临床疗效满意。
