血浆NT-proBNP、ATⅡ和IGF-Ⅰ水平衡量原发性高血压严重程度的临床分析 被引量:4

摘要 目的:为了探索血浆NT-proBNP、ATⅡ和IGF-Ⅰ水平衡量原发性高血压(EH)严重程度的临床分析。方法:荧光免疫分析、RIA和酶免疫分析分别测定265例EH患者血浆中的NT-proBNP、ATⅡ和IGF-Ⅰ水平并与65例正常对照组进行了比较。结果:265例EH患者的血浆NT-proBNP和ATⅡ水平较65例正常对照组非常明显增高(P均<0.01),而血浆IGF-Ⅰ水平非常明显降低(P均<0.01);36例血压控制达标组EH患者的血浆NT-proBNP、ATⅡ和IGF-Ⅰ水平较65例正常对照组无明显差异(P均>0.05);73例EH I级组患者血浆NT-proBNP和ATⅡ水平较65例正常对照组分别为无明显差异或明显增高(P>0.05~<0.05),而血浆IGF-Ⅰ明显降低(P<0.05);78例EHⅡ级组和78例EHⅢ级组患者的血浆NT-proBNP和ATⅡ水平较65例正常对照组均非常明显增高(NT-proBNP P<0.05和P<0.01;ATⅡP均<0.01),而血浆IGF-Ⅰ非常明显降低(P<0.01)。结论:EH患者的血浆NT-proBNP和ATⅡ水平与血压的临床分级呈正相关,而血浆IGF-Ⅰ水平与血压的临床分级呈负相关,表明EH患者的严重程度(血压的临床分级)与血浆NT-proBNP、ATⅡ和IGF-Ⅰ的水平密切相关,并可能参与EH的病理生理过程。 Objective In order to evaluate clinical analysis of exploring essential hypertension severity through determining plasma NT-proBNP,ATⅡ and IGF-Ⅰ levels. Methods Plasma NT-proBNP(with fluroimmunoassay),ATⅡ(with RIA) and IGF-Ⅰ(with enzyme immunoassay) levels were measured in 265 patients with essential hypertension(EH) and 65 normal controls as well as comparable studies. Results Plasma NT-proBNP and ATⅡ levels in 265 patients with EH were significantly higher then those in 65 controls(P all0.01),but plasma IGF-Ⅰ level was significantly lower(P0.01).Plasma NT-proBNP,ATⅡ and IGF-Ⅰ levels in 36 patients with blood pressure control normal group were tightly increased and lower then those in 65 controls(P all0.05).Plasma NT-proBNP and ATⅡ levels in 73 patients with EH Ⅰ stage were tightly elevated and significantly higher then those in 65 controls(P0.05~0.05),but plasma IGF-Ⅰ level was significantly lower(P0.05).Plasma NT-proBNP and ATⅡ levels in 78 patients with EHⅡ stage and in 78 patients with EHⅢ stage were prominently higher than those in 65 controls(P0.05~0.01),but plasma IGF-Ⅰ level was very marked lower(P0.01). Conclusion Plasma NT-proBNP and ATⅡ levels in patients with EH were positive correlation with EH clinical stage,while the same result was not found for plasma IGF-Ⅰ level.This was showed that the severity of EH patients was closely correlation with EH clinical stage,NT-proBNP,ATⅡ and IGF-Ⅰ were probably involved in the pathophysiological process of EH.
