常规受精和卵母细胞胞浆内单精子注射受精在获卵较少周期的临床结局比较

Comparison of clinical outcomes between conventional in vitro fertilization and intracytoplasmic sperm injection in low oocyte retrieval cycles

目的探讨体外受精—胚胎移植(IVF-ET)周期中,获卵数较少的患者受精方式的选择。方法选择在该院进行IVF-ET,女方获卵数≤3的夫妇做为研究对象。根据女方年龄分为≥35岁组和<35岁组,根据精液情况分为正常精液ICSI组和少弱精ICSI组,分别比较各组内的2PN率、卵裂率、临床妊娠率、着床率及活婴产率。结果对于获卵数较少的夫妇,不论女方年龄是否≥35岁,采取常规受精与采取单精子注射受精的2PN率、卵裂率、临床妊娠率、着床率及活婴产率均没有显著性差异。精液正常行ICSI和少弱精行ICSI的2PN率、卵裂率、临床妊娠率、着床率及活婴产率均没有显著性差异。结论 IVF治疗时,获卵数较少的夫妇单精子注射并不能改善妊娠结局,受精方式的选择仍以精液指标为准。

【Objective】 To investigate the method of fertilization during in vitro fertilization-embryo transfer（IVF-ET） in patients who obtained no more than 3 oocytes.【Methods】 Within the couples proceeded IVF-ET in our center,we selected the female had obtained oocyte number no more than three and her partner had normal sperm parameters as study objects.According to the female age,the couples were divided into two groups： the group with female age no more than thirty-five years old,and the group with female age more than thirty-five years old.The rate of 2PN,oocyte cleavage and clinical pregnancy were compared between these two different fertilization methods.【Results】 For the couples who had obtained less oocyte number,the rate of 2PN,cleavage and clinical pregnancy had no difference between the regular fertilization method and the ICSI.【Conclusion】 For the couples who obtained less oocyte number during IVF-ET,ICSI could not improve the rate of pregnancy and the method of fertilization should be selected according to the sperm parameters.
