腔内同期手术治疗输尿管结石合并前列腺增生效果评价

Simultaneous Endoscopic Treatment for Benign Prostate Hyperplasia and Ureteral Calculi

目的评价腔内同期手术治疗输尿管结石合并前列腺增生的安全性及应用价值。方法回顾性分析我院2005年8月—2010年6月收治的17例行腔内同期手术治疗的输尿管结石合并前列腺增生患者的临床资料。结果17例均手术成功,无中转开放手术,平均手术时间76.4 min,未出现大出血、输尿管穿孔、电切综合征等严重术中并发症,无围术期死亡病例,其中1例输尿管上段结石滑入肾盂,二期行体外冲击波碎石成功。术后2个月复查B超未见结石残留。术后6个月随访未出现永久性尿失禁、尿道狭窄等严重并发症,平均最大尿流率较术前明显升高,国际前列腺症状(IPSS)评分、生活质量指数(QOL)评分较术前明显降低,差异均有统计学意义(P<0.05)。结论腔内同期手术治疗输尿管结石合并前列腺增生是一种安全、有效的手术方式,是治疗输尿管结石合并前列腺增生的较好选择。

Objective To evaluate the safety and the application values of simultaneous endoscopic treatment for benign prostate hyperplasia and ureteral calculi.Methods Data of 17 patients who underwent simultaneous endoscopic treatment for benign prostate hyperplasia and ureteral calculi in our hospital between August 2005 and June 2010 were retrospectively analyzed.Results All operations were successful with an average operating time of 76.4 min and no transfer to open operation was made.No serve complications occurred except for one case in which the upper ureteral calculi migrated to the kidney pelvis and postoperative ESWL turned out to be successful.No residual calculi were seen two months after operation by B-ultra sonography.No severe complications including urinary incontinence and urinary stenosis were noted 6 months after the operation.Maximum urinary peak flow rate increased postoperatively while international prostate symptom score（IPSS） and quality of life score decreased with significance（P0.05）.Conclusion Simultaneous endoscopic treatment for benign prostate hyperplasia and ureteral calculi is both safe and effective.And it is a good choice for treatment of benign prostate hyperplasia and ureteral calculi.
