摘要
目的探讨经颅多普勒(TCD)和脑干听觉诱发电位(BAEP)对后循环缺血(PCI)的联合诊断价值。方法采用TCD和BAEP技术对128例临床诊断为PCI的患者进行检测,评价基底动脉(BA)、椎动脉(VA)、大脑中动脉(MCA)、大脑前动脉(ACA)、大脑后动脉(PCA)的血供情况,同时对BAEP各波潜伏期(PL)、峰间潜伏期(IPL)进行分析。结果 128例患者中TCD异常率为75.0%(96/128);BAEP异常率为68.7%(88/128)。TCD和BAEP总异常率为87.5%(112/128)。TCD、BAEP、TCD+BAEP 3种方法检出率比较,差异有统计学意义(P<0.01)。结论 TCD、BAEP在PCI诊断中各具优势,分别从神经电生理和血流动力学方面为PCI的诊断和定位提供有价值的客观依据,联合应用能更全面地反映病情,提高阳性检出率。
Objective To investigate the diagnosis values of combined transcranial doppler(TCD) and brainstem auditory evoked potential(BAEP) for patients with posterior circulation ischemia(PCI).Methods A total of 128 patients with PCI were examined with TCD and BAEP with the following indices evaluated,including BA,VA,MCA,ACA,and PCA.PL and IPL were also analyzed.Results Among the 128 patients,total abnormality rate of TCD and BAEP was 87.5%(112/128).Abnormality rate of TCD was 75.0%(96/128).Abnormality rate of BAEP was 68.7%(88/128).Abnormality rates among the three methods were significantly different(P0.01).Conclusion TCD and BAEP both have advantages in diagnosing PCI,providing objective evidences from the aspects of neuroelectricity physiology and hemodynamics respectively.Combined TCD and BAEP reflect the condition of the patients comprehensively,which results in higher abnormality rate.
