静脉输液港对60例乳腺癌患者的临床应用与分析

Clinical Application and Analysis of Venous Port in 60 Cases of Breast Cancer Patients

目的通过对60例乳腺癌患者应用静脉输液港的观察,找出并总结此类患者应用静脉输液港经常出现的问题,有针对性地进行防护和护理,更好地保证静脉输液港为乳腺癌患者提供有效的输液通路。方法通过对我院乳腺外科2010年1月到4月60例应用静脉输液港的乳腺癌患者的观察,找出常见问题进行总结分析,从而研究出有针对性的护理对策。结果 60例应用静脉输液港的乳腺癌患者有12例出现问题,其中8例出现注射底座皮肤红肿,占13.3%;2例出现穿刺点红肿,占3.33%;1例出现置管侧上肢肿胀,占1.67%;1例抽不到回血,占1.67%。结论护士经验不足,如穿刺方法错误、穿刺深度不够、穿刺位置不准确等;患者依从性欠缺,如不按时或忘记回院维护置管;管理欠规范,如空气和局部消毒无菌操作未做明确的规定等是造成乳腺癌患者留置静脉输液港出现问题的主要原因。对护士加强专业培训、对病人强化健康教育、完善管理制度,才能提高静脉输液港的护理质量,为患者化疗和其他静脉治疗提供有效的通路,从而提高患者的生活质量,体现优质的护理服务。

Objective By observation of 60 cases of breast cancer with venous port, to identify and summarize the patients with venous port of the recurring problems, targeted for protection and nursing, to better ensure venous port provide effective infusion pathway for breast cancer patients. Methods 60 breast cancer patients with venous port in our hospital from January to April 2010 were observed to identify and analyze the common problems for targeted nursing countermeasures. Results In 60 cases of application of venous por, 12 cases had problems, including 8 cases with injected skin irritation （13.3%）, 2 cases with inflamed puncture （3.33%）, one case with swelling in the tube side of the upper extremity （1.67%） and one case without pumping blood back （1.67%）. Conclusion Nurses with the lack of experience, patients with the lack of compliance, poor management standards are the main reasons of indwelling venous port. Enhancing professional training for nurses, strengthening health education for patients, improving the management system can improve the venous port nursing quality for patients, to provide an effective pathway for patients with chemotherapy and other intravenous therapy, so as to improve the quality of life for patients and reflect the quality of nursing.
